Order Mindy Jostyn CDs by Mail

Follow these instructions:

  1. Tell your browser to print this page. Then fill out the form and mail it with your payment.
  2. Fill in your name and shipping address.
  3. Enter each item you would like to order.
  4. Total the cost of your items.
  5. For billing addresses in New York, add 8% NYS tax by multiplying the total cost of items by 0.08 .
  6. Add shipping costs:
    for order to US: $2.50 for the 1st item, $1.50 for the 2nd item, plus $0.75 for each additional item.
    for order to Canada, and Mexico: $3.50 for the 1st item, $1.50 for the 2nd item, plus $0.75 for each additional item.
    for orders to other countries: $5 for 1st item, $4 for 2nd item, $1.50 for each additional item.
  7. Enclose your personal check or money order payable to: Moonboy Records
    Note: please make sure to include your phone number on the order form or your check.
  8. Mail the order form to the address below.
